Please email cms1500@StandardMedicalBilling.c-o-m (without the hypens) to request a userid and password to login and see the full patient records. State and Fedrally allowed charged apply to records requests. The patient account number from CMS 1500 box 26 and the patient's demographics and proof of the patient's authority to access their records will be needed to review individual medical charts. Additionally, a detailed description of the terms of each individual CMS 1500 is available on that site.

Medical records requests must be accompanied by payment in the amount allowed by state and federal law for preparation of the requested medical records and shipping of the requested records. Medical records are mailed using USPS priority mail Regional A box(es) or larger USPS priority mail boxes with a tracking number. Requests sent without payment may not receive a response at Standard Medical Billing sole discretion. For example, Maryland states "The fees that may be charged for preparation and production of medical records may be adjusted annually for inflation using the Consumer Price Index on July 1 of each year. Beginning January 1, 2014, adjusted rates for medical record copying are as follows: a preparation fee of no more than $22.88 (Please note that preparation fees can be charged to hospitals and insurance companies, but NOT patients.), plus a fee of no more than 76 cents per page copied, plus the actual cost of shipping and handling." at https://health.maryland.gov/mbpme/Pages/records.aspx. Thus, as of January 2017, each and every request for records must be accompanied by a check (if emailing or faxing request then send payment via PayPal to info@StandardMedicalBilling.com) for at least $30.27 ( = $23.75, which is $22.88 with CPI adjustment, + $6.52 for USPS shipping to closest zone). When the records are sent (up to the number of pages that were paid for), a bill for any additional actual shipping costs and the allowed per page fee and any other allowed fees will be included. For example, a request accompanied by the minimum payment for a record of unknown length would would be responded to with the first page of the record and a bill for the balance needed to be prepaid to cover the remaining pages. Please note that Maryland specifically states that "preparation fees can be charged to hospitals and insurance companies". A medical records request form is available
